The post holder will work as part of the integrated 0-19 children’s team and will support the team in the delivery of a core programme of health care for children, young people and their families. This will be within a variety of venues under direct and indirect supervision of the Specialist Community Practitioner (SCP). The post holder will be designated named nurse for a number of schools.

The post holder will work within a clinical governance framework, which is embedded in the day-to-day practice. Every clinical member of staff must work within the framework
COMMUNICATION AND RELATIONSHIPS
The post holder will be required to:
• establish clear and effective communication links with children/young people, parents/carers, education staff, and a range of other professionals from both voluntary and statutory organisations.
• Listen appropriately and take into account any sensitive issues and communicate these to appropriate persons/agencies ensuring that children are protected and that disclosures are acted upon appropriately
• Utilise a range of communication skills eg, negotiation to gain commitment from others and formal presentations, etc
• Feedback the views and needs of children/young people, families and schools to the Specialist Community Practitioner as necessary, to enable appropriate interventions to be implemented.
• Utilise a range of communication methods with colleagues and managers to include electronic communication
ANALYTICAL AND JUDGEMENTAL
The post holder will be required to:
• the post holder will be able to recognise their own level of professional competence and identify sources of support as required
• participate in clinical supervision sessions within the designated locality.
• assess, plan and implement interventions and programmes of care for children and young people, and discuss with the SCP as appropriate.
• use their own professional judgement to identify solutions to a range of day-to-day problems, seeking advice where appropriate from the SCP.
• identify the relevant health and social issues within the designated caseload and report to the SCP This will include the identification of issues relating to safeguarding children and vulnerable adults.
• deal with situations where he / she will need to assess events, problems or patient condition in detail to determine an appropriate course of action. An example of this may be problem solving and fault finding on familiar equipment, making appropriate referrals as necessary
• assist in the provision of education and support necessary to ensure learning occurs following accidents and incidents
PLANNING AND ORGANISATION
The post holder will be required to:
• manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines.
• assist with the planning and organisation of immunisation sessions.
• respond to unexpected changes in staffing and clinical issues
• manage unpredictable and/or volatile situations effectively.
• act as a mentor to pre-registration students as appropriate and be involved in organising workload of student nurses and junior/new members of the team
• undertake mandatory training as required by the and be responsible for own professional updating and keep abreast of current research based practice.
• identify and refer any potential and actual risks involved in work activity to the
Specialist Community Practitioner and assist in addressing and managing
such risks
PHYSICAL
The post holder will be required to:
• carry materials and equipment, eg weighing scales, Leicester height measure, boxes of leaflets, audiometers, immunisation equipment etc in accordance with relevant health and safety PCT policies
• sit at a computer in order to input data on to systems which would require accurate keyboard skills.
• drive and hold a current driving licence and have access to a vehicle for work and to be able to travel efficiently around the local geographical area. Provide evidence on an annual basis that the vehicle is insured for business use
• move around effectively to undertake the roles and responsibilities of the post
• have the accuracy and dexterity to participate in public health sessions such as immunisations, growth monitoring and screening.
PATIENT AND CLIENT CARE
• undertake health surveillance and screening as part of an agreed core programme of care, referring to appropriate services as necessary.
• assist and participate in the planning and delivery of immunisation sessions in schools/clinics.
• participate in the Health assessment of Children who are Looked After (CLA), seeking advice and support from CLA Co-ordinator as appropriate.
• undertake health promotion/education activities that are congruent with identified needs and local and government targets; seeking advice from the Specialist Community Practitioner as appropriate.
• demonstrate an evidence-based approach to practice to enable the development of a high standard of practice.
• demonstrate cultural awareness in relation to client needs.
• undertake some home visits where necessary, and in accordance with the ’s Lone Workers Policy.
• provide designated preschools, nurseries and schools with advice and training/awareness on health related matters as appropriate; seeking support and guidance from SCP as necessary.
• plan and participate in the Department of Health initiatives as required.
• utilise community profiles to identify health needs and plan programmes based on identified needs.
• maintain professional records in accordance with the and NMC Record Keeping Policies.
• work in accordance with Local Safeguarding Children’s Board’s procedures and participate in Safeguarding Supervision. With appropriate training and guidance, recognise indicators of child abuse, reporting concerns to senior member of the team. Participate in identified work with families, children and young people, attending case conferences, core groups and preparing reports as required.
POLICY AND SERVICE
The post holder will be required to:
• work in accordance with NMC Code of Professional Conduct
• adhere to the ’s policies and guidelines and to participate in appropriate research and audit in order to influence local and national policies ensuring research based practice.
• recognise ethical and legal issues which have implications for health care of children, young people and their families and take appropriate action
• work within the framework of the local Safeguarding Children policy and guidelines to promote the health and safety of all children in partnership with parents and other agencies
• Be involved in the development of evidence based practice and to ensure that practice is current and dynamic responding to clinical evidence as it develops. To initiate and support innovative change within Children’s Services in line with current clinical and academic evidence, national standards and relevant reports
• attend relevant and team meetings. Have an understanding of the importance of national and local health initiatives and contribute towards achieving specified targets
FINANCIAL AND PHYSICAL RESOURCES:
The post holder will be required to
• be financially aware in order to manage the ’s resources appropriately and effectively.
• Be responsible for ensuring that equipment is maintained in accordance with policy.
HUMAN RESOURCES
• maintain up-to-date professional registration with the Nursing and Midwifery Council (NMC). Provide evidence on an annual basis to this effect.
• provide the mentoring of pre registration students and support the team in their responsibility for teaching, supervising and students and other staff.
• take responsibility for own professional development and identify training needs.
• support team members actively participate in individual Knowledge and Skills Framework Programme.
INFORMATION RESOURCES
• demonstrate accurate keyboard skills with working knowledge of Word and Outlook.
• accurately record all data and client contacts in accordance with policies and procedures, in light of Caldicott principles and Clinical Governance (including information that may be required for Safeguarding Children, audit and research purposes).
• ensure all records are accurate, contemporaneous and updated regularly, according to local and national guidelines

Registered General Nurse
Current Registration with NMC Mentorship/preceptorship qualification or willingness to undertake
