Main responsibilities-Community Staff Nurse job description
1. PROFESSIONAL
* 1.1 Work in accordance with Code of Professional Conduct and scope of Professional Practice.
* 1.2 Provide nursing services to patients within their own home at GP premises/health centres or other community setting in liaison with other members of the team.
* 1.3 In conjunction with the Team Leader use nursing models to assess, plan, implement and evaluate allocated patients needs.
* 1.4 Participate in identifying the health needs of the local population.
* 1.5 Provide support and advice to patients and informal carers, including the teaching of basic nursing care.
* 1.6 Work to professional and service standards and participate in the auditing of these standards when appropriate.
* 1.7 Participate in the promotion of healthy living and the prevention of ill health and disability as a priority of care in the community.

2. MANAGEMENT
* 2.1 Participate in the management of all available resources to ensure an effective and efficient service, which is consistent, accessible and relevant to patients needs.
* 2.2 Responsible for ensuring accurate and timely information is provided for the data collection system.
* 2.3 To maintain clear, accurate and contemporaneous records according to guidelines for Clinical Record Keeping.
* 2.4 Take responsibility for the management of the caseload in the absence of a qualified District Nurse.
* 2.5 Work in collaboration with GPs, Social services and other agencies to provide comprehensive packages of care.
* 2.6 Advise team leader of any circumstances which give cause for concerns in relation to patient care

3. EDUCATION AND DEVELOPMENT
* 3.1 Keep up-to-date with current knowledge and advances in patient care.
* 3.2 Participate in providing community experience for student nurses and other personnel on secondment.[cont right]

* a) The Trust Policies and Procedures
* b) Health and Safety regulations and Fire Regulations
