To work as a member of the Home Treatment Team providing a seven days per week, twenty four hours a day, working flexibly across the full span of service hours.

3 COMMUNICATION AND WORKING RELATIONSHIPS

4. PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY-Mental Health Nurse job description
• Provide a service, which is in line with the team Operational Policy, i.e. client focused, accessible and responsive on a 24-hour basis.
• Participate in the assessment of clients who are appropriate to be seen by the team and undertake the development and implementation of treatment plans, working within the agreed supervision arrangements within the team.
• Continually review and assess clients needs and to seek supervision and guidance as appropriate.
• Provide information, education and support to family members, carers and other professionals.
• Contribute to the education of other staff and students.

b) Commences individual care planning (including discharge planning) during initial entry and assessment phase and clearly documents interventions and plans.
c) Considers the biological, psychological, occupational, spiritual and social needs of care and provides the client and their carers with adequate feedback appropriate information regarding care.
d) Provide appropriate information to other team members of the multi disciplinary team and relevant care providers to ensure that the Care Plan established for the client continues when others provide care.

Occupational Therapists
• To use Occupational Therapy Skills within the role of Key Worker.
• To provide Occupational Therapy specific interventions.
• To provide Occupational Therapy advice to other members of the team.
Social Workers
• To provide Social Worker specific interventions.
Nurses
• To monitor and provide advice on matters related to medication.
• To administer medication to people in crisis to alleviate acute symptoms including administration PGD medication (patient group direction) in the absence of an available Doctor as per policy.
• To provide Mental Health Nursing specific assessments and interventions.
6. DECISION MAKING & ADVICE
• Proactively address areas of concern and report any areas of concern to the Team Manager.

8. HEALTH, SAFETY AND RISK MANAGEMENT
The post holder must at all times comply with the Surrey and Borders Partnership Foundation Health & Safety Policies, in particular by following agreed safe working procedures and reporting incidents using the ’s risk incident reporting system.
The post holder is required to inform the line manager of any safety issues that could affect you or others in the work place.
You are responsible for your own health & Safety and must co-operate with the management at all times in achieving safer work processes and work places, particularly where it can impact on others.
You will be trained in the correct use of any equipment provided to improve Safety and health within the . You are required to use the equipment when necessary and as instructed; it is your responsibility to ensure the equipment is safe to use, prior to its use and must report any defects immediately to your manager.
9. TRAINING AND PERSONAL DEVELOPMENT
The post holder must take personal responsibility in agreement with their line manager for his or her own personal development by ensuring that continuous professional development remains a priority.

ESSENTIAL
Qualifications
• Relevant core Professional Qualification – i.e. Diploma OT/BSc OT, Dip SW, RN (Mental health), or other relevant qualification.
• Evidence of post qualification development.
Experience
• A minimum of two years experience of working with adults with acute mental health problems.
• Experience of supervising other staff.
• Experience of working with people at risk.
• Can demonstrate experience of formulating care plans for people in acute mental health crisis.
Competencies
• Assessment and problem solving skills, particularly in the area of crisis assessment and treatment.
• Ability to consult, liaise and negotiate with service users, carers/family members, other professionals and the wider community.
• Ability to work within a multi-disciplinary team.
• Able to demonstrate evidence-based practice.
• A commitment to the delivery of services that are responsible to client’s culture, gender and families.
