• To Triage patients attending the Urgent Care Centre and determining patient priority

To see and treat patients in the Urgent Care Centre with complex needs prior to consultation with a clinician

Main responsibilities- Triage Nurse job description

1.0 Clinical
1.1 To triage walk-in patients at the Primary Care Centre, treat and discharge or refer patients to a clinician using decision support software.

1.2 To provide clinical care in the PCC and community
1.3 The ordering, storage, administration and disposal of medicines in a safe, secure manner in accordance with current legislation, licensing requirements and best practice

1.4 To reassure the rapid and effective transmission of patient data between all relevant parties

1.5 To deal with child protection issues

1.6 Promote evidence-based practice

1.7 Establish and maintain strong collaborative arrangements with other health care professionals

1.8 Participate in admission avoidance activities

1.9 To participate in the Emergency Department dive
2.0 Managerial
2.1 To act as shift co-ordinator delegating staff making the best use of their abilities.
2.2
To participate in the appraisal process for themselves and others

2.3 To diffuse challenging behaviour, ensuring that the situation is managed in a sensitive way.

2.4 To implement disciplinary and grievance procedures if required
2.5 To inspire others and encourage them to seek advice and solutions to problems.

2.6 To challenge others to take an active part in developing knowledge, ideas and work practice

2.7 To challenge tradition and take risks accepting joint responsibility for any arising problems and tensions and using these to inform future practice

3.0 Education & Training
3.1 Participate in and facilitate and open, reflective and learning environment.

4.0 Professional
4.1 Exercise individual responsibilities and accountability for own practice, in accordance with clinical governance and professional guidance from NMC.

5.2 To work together with the complaints team to address user complaints.

6.0 Responsibilities for physical and financial resources
6.1 To ensure efficient and effective use of material resources/supplies within the centre
6.2 To identify any problems with resource use/availability and make recommendations for corrective action
6.3 To maintain accurate records of resource use

7.0 Communications
7.1 To communicate with a wide range of people to explore complex issues and to make complex decisions
7.2 To effectively communicate complex and sensitive information.
7.3 To agree the arrangements for communication with the patient/family/carer, and to document these in accordance with policy and the patients right to confidentiality

8.0 Analysis and data management
8.1 To undertake data collection effectively using the agreed systems.
9.0 Partnership working/service development
9.1 To work in an integrated and seamless way with other colleagues and disciplines within the Primary Care Services, including Intermediate Care and the Emergency Department of UHL.
9.2 To adhere to policies and procedures relating to own workplace to contribute to service development
9.3 To implement and propose changes to policies/practices in own area
9.4 To evaluate with others the effectiveness of any changes and how these have improved services
9.5 To develop, sustain and evaluate collaborative working relationships with acute and primary care agencies

Knowledge, Skills and Experience- Triage Nurse job description

QUALIFICATIONS/ TRAINING

Current First Level registration
Diploma Level Health Related Study

SKILLS/ EXPERIENCE
Minimum three year post registration (Minimum of one year in the primary and/or community and/or acute settings)
Evidence of working independently
Excellent interpersonal and communication skills.
Demonstrates ability to engage patients and the public in the development of the service
Demonstrates ability to work as part of a team
Has a working knowledge of venerable adult and children issues.

SPECIALIST KNOWLEDGE
Awareness of Equality & Valuing Diversity Principles

Understanding of Confidentiality & Data Protection Act

APTITUDES

Basic computer skills
Evidence of professional profile
Evidence of previous multi-professional working
Good written and verbal communication skills

DISPOSITION

Is able to meet the needs of the service flexibly
